What are we looking for in a Care Coordinator?

Care Coordinators on the WISe team provide wraparound services to youth and their families, including collaborative and creative problem-solving to support clients in meeting their treatment goals. The Care Coordinator develops and facilitates individualized support plans and acts as liaison with other providers. Treatment often takes place in the client?s natural environment during times that are convenient for the family, sometimes outside of regular business hours. Care Coordinators must be skilled in managing team conflict, engaging a variety of personalities, facilitating meetings, and communicating with various audiences.


Care Coordinator duties may include:

  • facilitating meetings between the client, family, and treatment team
  • developing a collaborative treatment plan
  • providing crisis evaluation and intervention
  • engaging clients and team in treatment
  • following WISe protocol with fidelity to the model
  • completing all documentation using strengths-based language
  • providing services outside of regular business hours, including frequent after-hours calls

Qualifications:

  • Bachelor's degree in social work, psychology, social services, or related field
