The Safety Net Medical Home Initiative was a five-year (2008 to 2013) national Patient-Centered Medical Home (PCMH) demonstration project to aid 65 primary care safety net sites become high-performing medical homes and improve quality and patient experience.
The Initiative, led by Seattle-based quality improvement organization Qualis Health, laid the groundwork for PCMH transformation and published a library of resources and tools to help practices implement the PCMH Model of Care.
